In our supported living programs, we provide high quality, CARF-accredited, services for those with the Community Integration and Habilitation Waiver. Based on the person-centered-planning philosophy, we provide what is needed, when it is needed. That can be a few hours to help with daily activities such as banking and grocery shopping or it can be 24-hour care; we can create a schedule and plan based on the resources available that will meet the needs of the person.
We work closely with each person’s support team to create an optimal plan. Team members would include the person receiving services, their family members, any advocates and a case manager. Teams also can include physicians, nurses, medical specialists, behavior specialists, etc., depending on each person’s needs and preferences.
Carey Services can provide care ranging from recreational opportunities, healthcare coordination, financial management, medication administration to advocacy, etc.
We provide supported living services to Wabash, Grant and Blackford counties.
Group Homes
Carey Services owns and operates group homes in Blackford and Grant counties. Group homes provide 24-hour supervision for six to eight people living in a home setting. Qualified and caring staff provides the necessary support and training to allow each person living in the home opportunities to learn new skills enabling them to become as independent as possible.

Group home staff members are highly trained in universal aspects of person-centered consumer care such as CPR, nutritional need and medication administration as well as the unique medical and behavioral needs for each person.
Group home residents vary in regards to age and activity level to ensure a good fit for each person who lives in the home.
